The first step is a consultation with our specialist. You will discuss your personal and family history, past illnesses, injuries or surgeries. A physical examination of the reproductive organs may also be part of the consultation.

Step 2: Semen Analysis – The Cornerstone of Diagnosis

Semen analysis is a key laboratory test that evaluates the quality of the ejaculate.

Sample Collection:

The sample is obtained through masturbation into a sterile container in a completely private and discreet collection room directly at our clinic. If this is uncomfortable, the sample can also be brought from home.

Laboratory Analysis: In the lab, we assess key parameters:

  • Sperm count (concentration)
  • Sperm motility
  • Sperm morphology (shape)
  • Sperm vitality
  • Possible presence of infection and other factors

Because sperm quality can naturally fluctuate, a repeated test may be needed over time for more accurate results.

Step 3: Additional and Specialized Tests

If the basic test reveals a problem or the cause remains unclear, our doctor may recommend further tests:

  • Blood tests: To determine hormone levels (testosterone, FSH, LH), vitamin D.
  • Testicular ultrasound: A painless examination to detect structural problems (e.g., varicocele).
  • Genetic tests: Can detect chromosomal defects or specific mutations (e.g., cystic fibrosis) causing infertility.
  • Immunological testing: Detects antibodies against sperm.
  • Testicular biopsy: A microsurgical procedure to obtain a tissue sample directly from the testicle. This is done when no sperm are found in the ejaculate (azoospermia).

Most Common Causes of Male Infertility

The causes of male infertility can be divided into three main types: pretesticular, testicular, and post-testicular causes. Male fertility is also affected by genetic and immunological factors.

Pretesticular Causes

These involve conditions where infertility is caused by insufficient development of the male gonads – the testes. This can be due to a variety of reasons, such as genetic abnormalities, hormonal changes, and more.

Testicular Causes

These cases involve poor-quality ejaculate in terms of the number of viable and motile sperm. Sperm may have poor shape or composition (teratospermia). A low sperm count is called oligospermia. The complete absence of sperm in the ejaculate is called azoospermia.

Numerous factors can affect ejaculate quality, including past infections such as mumps, malaria, testicular tumors, and other diseases.

Posttesticular Causes

In men with posttesticular causes, testicular function is normal, but sperm cannot exit the body. This may be due to blocked or absent vas deferens, prostate inflammation, urethral malformations, impotence, and more.

Genetic Factor

This applies to both men and women who have or are carriers of a genetic disorder. These cases may not necessarily involve inability to conceive naturally, but IVF is recommended to avoid the risk of genetic disease in the child. Immunological Factor
