Infertility treatment

Determination of appropriate treatment

Based on the initial consultation, the severity of the cause of infertility of the couple is assessed and the recommended treatment is developed. For couples where the diagnosis allows it, we proceed from simple treatment methods to the more demanding ones.

Treatment limitations

According to the valid legislation of the Czech Republic, we can only treat heterosexual couples who meet the age limit for women for the treatment with assisted reproductive methods. On the day of embryo transfer or insemination, the woman must not be older than 48 years + 364 days. At the age of 49, treatment can no longer be carried out.

BMI Limits

  1. Patients with a BMI over 35 have a significantly reduced success rate in assisted reproductive technology (ART) treatment
  2. In overweight patients, oocyte collection is usually technically difficult – the ovaries are poorly available for the aspiration needle and oocyte gain may not be adequate.
  3. When attempting to aspirate poorly available oocytes, there is a greater risk of injury to the surrounding organs (intestines, large vessels, bladder). In the case of such complications, an urgent intervention in the operating theatre is necessary, but we are not able to provide this in our department.
  4. General anaesthesia is very risky in overweight patients.

For the above reasons, our department has a set BMI limit of 40. If the patient exceeds this limit, oocyte collection may not be performed.

Types of infertility treatment

Following the diagnosis, the physician recommends appropriate treatment. Types of treatment depending on whether there is a problem on the part of a woman or a man are as follows:

  • Ultrasound monitoring of follicular growth followed by ovulation timing with a single hormonal injection with recommendation of unprotected sexual intercourse after 24-48 hours.
  • Intrauterine insemination (IUI)
  • In vitro fertilization (IVF)
  • In vitro fertilization (IVF) with donated eggs
  • In vitro fertilization (IVF) with donated embryo
  • In vitro fertilization (IVF) with ICSI
  • In vitro fertilization (IVF) using sperm from TESE
  • In vitro fertilization (IVF) with donated sperm
  • In vitro fertilization (IVF) with donated embryo
